In this instance we'll select Add Contact Field > Single Line Text > Add Custom Text, in this case 'City & State' > Add Fields to your forms > Select if Required > Add. Once Added you can rearrange the form order at Settings > Client & Contact Info, here you can click on the slider on the left of the forms icons to adjust the order.
