This will allow you to add a contact form integrated with Thryv Business Center to any website, including Wordpress, Wix, or GoDaddy, to capture important client contact information

1. Click on Online Presence

2. Click on Contact Form

3. Click on Customize

If you have already customized your contact form, you can skip right to Add To Website. Just copy the code provided and paste at the bottom of your HTML

4. Edit the Layout and Settings for your Form

From here, you can choose one of the following layouts:

*Basic Layout: standard contact form

*Contact & Profile: contact form with your profile information

*Contact & Schedule: contact form with a calendar button

*Professional: contact form with profile information and calendar option

You will see the form preview on the right

You can also edit the form fields and the colors/fonts

5. Click Add to Website

6. Copy and Paste the code to the bottom of your HTML page
