Start at your Dashboard
1. Click on Settings
2. Click on My Services
3. Select the desired service to edit
4. Scroll to the bottom of the service and select "Add/Edit" to add Additional fields
5. Select the desire field option
6. This example shows the Single line field
Add the Label of the field, select if the field is required, then click Save
7. Repeat for more fields if needed
8. This example shows the Dropdown option.
Enter the Label name, the dropdown options, check if the field if required, then click Save
9. You will now see your additional questions
10. Click on Save to save your changes
11. All of your additional information will show on the appointment details
