Any form / client card fields are not appearing to any returning clients that have filled out a form previously. Currently, if someone scheduled an appointment and comes back another time to schedule again the form fields /client card fields do not appear.. only Notes appears and a link to Enter a different email. If "Enter a different email" link is clicked then form fields appear. Ideally, we need all form fields to always appear for returning clients.
We should either show full form / all fields to returning clients

• it assumes all fields from first request are same and this is a major problem.
